Xbox One Continues to Disconnect From Controllor

Thanks for coming to the forums. Sorry for the issue with controller disconnecting. Since you have already tried updating the controller's firmware, I would suggest checking for any updates on your console. Also, I will list below some other solutions that can help with the issue. Give it a try and let us know the results!
Restart your controller

Power off the controller by pressing and holding theXbox button for 6 seconds.

Press theXbox button again to power it back on.
Check for a system update:
To see if there's an update available:

Press theXbox button to open the guide.

SelectProfile & system >Settings >System >Updates.

If there are any updates, they'll appear in the left menu. If not, you'll see "No console update available."
Reset your console: Select System > Settings > System > Console info > Select Reset console > Choose Reset and keep my games and apps.
Note =>The "Reset and Keep my Games and Apps" option will not cause losing your games, but please be aware that you need to be completely aware of the email address as well as the password of your account before going for it because it will require you to sign in to the account.

Hi! My name is Pam. I'm an Independent Advisor and I'll be glad to help you today.
Do you have an external hard drive plugged in to the console right now? Or any other wireless devices that can interrupt the signal? If yes, can you unplug it and Please Turn off console > (press and hold power button for 10 seconds) unplug everything from the console > wait for 2 minutes and plug everything back > Turn On the console.
Once done, please check if the issue still persist while the external hard drive or any other wireless device is unplugged.

You can also try this: Press the Xbox button to open the guide.
Select Settings > System > Console info > reset then keep my games and apps.

Considering that your controller works fine with your PC, most likely it has something to do with the console. Can you try other USB ports on your console? If the issue still persist, I suggest to request for a service. I totally understand that you are having issues with the console. No worries, I have here the link to request a service for your console, make sure to register it first:

Once registered, click show details then Start a service online.

I've found a resolution for my problem. If I plug my controller into my USB switch the controller doesn't disconnect, even though the other inputs do disconnect randomly! When I swap the inputs to my PC everything works perfectly and the Xbox controller continues to work on the Xbox (weirdly).
